What is Early Intervention?

If your child is under the age of 30 months and his/her pediatrician recognizes signs of a developmental delay, your child will be referred to the Early Intervention program. A service coordinator will then be assigned to your family to help you get the help your child needs. He or she will perform evaluations so that more detailed information can be reviewed about your child.

An evaluation is the process of getting information about your child to assess his/her developmental needs and provide quality services and support to your family.

How to Get Your Reward for Early Intervention

Your family can earn $200 when you complete a pediatrician-advised Early Intervention referral and evaluation for a child under 30 months. Each of your children in this situation can earn the $200 Reward once per year.

What to send in:

1. Coupon. Submit the Early Intervention coupon from your Coupon Book.

2. Pediatrician-Advised Early Intervention Referral and Evaluation Form. Your service coordinator must completely fill out the Pediatrician-Advised Early Intervention Referral and Evaluation form following the coupon in the Coupon Book. Only completed forms will earn Rewards.
